What do I need?
- -Proof of Residency (current electric, water, or lease)
- -State certified Birth Certificate (not the “feety” one from the hospital)
- -Current Immunization Record
- -Child’s Social Security Card
- -Parent or Guardian’s valid driver’s license (address should match the Proof of Residency address)
- -Qualification Documents
